Factors affecting the treatment of vitiligo

Vitiligo treatment often fails for several reasons, let's list the most common:

1. Indiscipline: Irregularity in the use of medications and phototherapy. Intermittent and irregular treatment loses its effect.

2. Bad definition of the treatment by the doctor: Often only one cream is prescribed and that without prescribed phototherapy (UVB light with a wavelength of 311nm). Other times, a low defined number of radiations and the treatment is terminated prematurely. In both cases, vitiligo treatment is doomed to failure.

3. Absence of UVB light/sun: Many patients do not use light during treatment and rely only on creams or tablets. Light is an essential part of stimulating the formation of new pigment.

4. Short irradiation/phototherapy time: Incorrect duration of phototherapy leads to loss of effectiveness. Vitiligo does not burn with short exposure to light, erythema (redness) does not occur and new pigment does not start to form.

5. Associated autoimmune diseases: If there are associated autoimmune diseases that are not treated, it may mean that the progressive phase of vitiligo continues. We do not always know about the presence of an associated disease, so it is good to have the recommended examinations.

6. Lifestyle: Lifestyle, including factors such as stress, can affect immunity and thus the treatment of vitiligo.

7. Too aggressive forms of vitiligo: In some cases, vitiligo can be too aggressive and no form of treatment may be effective.

8. Financial constraints: Financial constraints may prevent some patients from accessing more expensive forms of treatment such as UVB phototherapy. In these cases, we recommend combining the treatment with the sun.

9. Age of the patient: Age can affect the success of the treatment. Vitiligo in younger people responds better to treatment than vitiligo in older people.

10. Extent of involvement: More extensive involvement is more difficult to treat and requires whole-body UVB phototherapy.

11. Late treatment: ignoring the disease leads to the loss of the memory trace of melanocytes (pigment cells) and the process of repigmentation is then longer and more demanding. In general, fresh vitiligo repigments quickly and therefore we do not recommend postponing treatment until later.

Vitiligo is still one of the diseases that cannot be completely cured and for which there is no miracle cure. Treatment of vitiligo is complex overall and requires cooperation between the patient and the doctor. The key element of success is discipline and a correctly determined treatment procedure.
