Your Vitiligo Treatment Plan

This is the protocol in calendar form – what to do on which day, and what to check when. It is the schedule our founder used and the one we send to customers who ask "what exactly do I do?". Background and reasoning are in the 7-step protocol; dosing details in the phototherapy guide. Agree the plan with your dermatologist, especially for children.

Before day 1

  • Dermatologist visit: confirm vitiligo (Wood's lamp), ask for blood tests – TSH, vitamin D, B12, folate, glucose. Treat what is out of range.
  • Photograph every patch in daylight. Same place, same light, once a month from now on.
  • Have ready: a 311 nm UVB lamp, Vitistop gel, Vitistop tablets (Vitix for children 3–11), vitamin D3, SPF 50+ sunscreen, goggles.

Every day

WhenWhat
Morning 1 Vitistop tablet after breakfast · vitamin D3 (autumn–spring) · prescription cream if prescribed (not on lamp mornings)
Before going out SPF 50+ on every exposed patch
Evening Prescription cream if prescribed · remove camouflage · moisturise

Lamp days – Monday, Wednesday, Friday (months 1–3)

  1. 60 minutes before: thin layer of Vitistop gel on the patches.
  2. Session: goggles on; lamp directly on the skin with the comb spacer.
  3. Week 1: 20 s per area. Then add 15–20 s per session.
  4. Target: faint pinkness a few hours later that is gone by next morning.
  5. Maximum 4–5 minutes per area; around the eyes maximum 1 minute.
  6. Redness lasting 2+ days → reduce by 45 s and hold.
  7. After: SPF 50+ if you go outside. No sunbathing on lamp days.

From month 4: Monday and Thursday only. After the patch has filled: once a week for 2–3 months, then stop, keep the daily routine.

Check-points

WhenWhat to expectIf not
4 weeks Patch edges pink after sessions; no new patches Check dose (pinkness?), film removed from lamp, sessions regular
6–12 weeks First brown dots around hair follicles or colour from the edges Increase dose to target; add prescription cream; check thyroid/vitamin D results
3 months Dots merging on face/trunk; 2–4-week tablet break, then continue Review with dermatologist; compare photos, not memory
6 months Small patches closing; hands/feet showing first dots Consider Vitipro gel; check lifestyle factors (sleep, stress, smoking)
12 months Most patches substantially repigmented; taper lamp Hands, feet, lips may stay partial – decide with your dermatologist on surgery or camouflage

Summer variant

Replace lamp sessions with 10–15 minutes of midday sun on the patches, three times a week, Vitistop gel 60 minutes before, SPF 50+ afterwards. Never sun and lamp on the same day; never let a patch burn.

Children

Same schedule with Vitix tablets (ages 3–11), tacrolimus 0.03 % instead of 0.1 %, an adult operating the lamp and shorter first exposures. See Vitiligo in children.
